1. 2010 NCFlex Annual Enrollment
   • Annual enrollment will begin October 5th, 2009 and continue through October 30th, 2009. You may not login prior to October 5th, 2009.
   • The enrollment site is www.ncflexonline.org
   • Your username and password are not connected to your login information with Appalachian. If you have forgotten your username and password then click on “Forgot Username, Forgot Password” it will prompt you on how to establish a new password or username.
   • You must re-enroll in your flexible health care spending account or dependent day care spending accounts for 2010. They do not automatically roll from year to year. If you do not re-enroll then you cannot participate in the 2010 year.
   • If electing dental coverage for the first time or if you choose to change your plan from low-option to high option, please read the waiting period clauses carefully. It is very likely that there will be a 12 month waiting period for any high option services.
   • All other benefit elections will roll-over from year- to- year. If you wish to change, add or cancel any of your existing flex benefits then you must log-in to the system and do so.
   • This NCFlex annual enrollment does not allow you to make any changes to your existing medical coverage. That is a separate enrollment period that occurs in March of each year.
   • More information during the annual enrollment period will be released. Check your email in the upcoming weeks for more helpful tips and additional information. Please contact the benefits office if you have any questions after reading your 2010 NCFlex booklet.
2. AFLAC Open Enrollment
   • All changes and sign ups must be done by October 23, 2009
   • Offering life, cancer, disability, accident, and other plans

   • Postponed from March 2009 to May 2009
   • Effective Date will be July 1, 2009

8. Donate Leave Time to an Employee in Need (Minimum 4 hours)

Time sheets can be accessed from the Payroll link under Appalachian State Electronic Forms . Choose the link entitled ASU Subject to Overtime Employee Banner Time Record.
